Reminders
If you’ve lost the consent form for a school trip you can use the blank one available from the list below or send in a handwritten note. Please ensure the note includes:-
- Trip name / destination (e.g. botanic gardens trip)
- That you give permission for you child to travel by car / coach, as appropriate
- For travel by car, whether you give permission for your child to sit in the front seat, back seat or in a seat with a lap belt
- For travel by car, confirmation that you will bring in a car seat or that a car seat is not required
